Please bring your insurance card/information.

A parent or guardian must accompany all patients under 18.

Please alert our office if you are taking any medications including blood thinners, bisphosphonates, antibiotics, etc.

Please inform the office if you have any medical conditions (high blood pressure, diabetes, artificial heart valves and joints, osteoporosis, heart attack, etc.).

Please alert our office if you require medication prior to dental procedures (ex. antibiotics after joint replacement surgery).

Please alert our office if you have any allergies to medications or latex.
